Please provide mailing and contact information
yourself as well as for the individual we should notify about
your gift.
If the gift is being given as a joy or tribute
gift, please indicate the name of the individual who the gift
is honoring and the occasion. We will send a personalized
card to your recipient notifying and thanking them for the
thoughtful gift given on their behalf and will also acknowledge
your gift directly to you.
