Boone County Assertive Community Treatment (BC-ACT)
"Treatment without Walls"
This project is funded by a grant from the Missouri Foundation for Health.
The BC-ACT programs is a collaborative project to enhance and expand capacity and strengthen integrated treatment for Adults with Co-occurring Substance Abuse and Mental Disorders (COD). Phoenix Programs is collaborating with Welcome Home, a halfway house for veterans, the Shelter, a residential facility for women who are victims of domestic violence, New Horizons, a psychiatric rehabilitation agency; Boone County Probation and Parole and the Boone County Jail to implement an Assertive Community Treatment project in Boone County, MO, (BC-ACT). This project will
provide comprehensive community based treatment to persons with severe and persistent mental illnesses and substance abuse disorders.
The BC-ACT treatment team will work collaboratively to deliver the treatment, rehabilitation and support services required by each client to help project participants live safely and autonomously in the community. The team will provide these services 24/7.
This project was created in order to fill a treatment gap in Boone County by ensuring treatment for those persons with COD who fall through the cracks or drop out of treatment programs because the traditional system is an inappropriate model for treating their severe, multiple symptoms.
